MCP server drops the public tracking tools and describes the rails tools

The BlindPay MCP server no longer lists the three public tracking tools, and two tools that had one-word descriptions now explain themselves.

Removed tools

GetV1EPayoutsById, GetV1EPayinsById and GetV1ETransfersById wrapped the /v1/e/ endpoints behind BlindPay's shareable tracking pages. Those endpoints only answer requests from the tracking pages themselves, so a call from an MCP client always failed. Use the authenticated tools instead, which return the same payout, payin or transfer:

Better descriptions

GetV1AvailableRails and GetV1AvailableBankDetails now describe what they return and how they fit together: list the rails first, then pass a rail's value to bank details to get the fields, validation patterns and picklists a bank account needs for that rail.
